Sending out Customer Account forms for completion - Docusign or alternative

Hi, our business would like to send out credit account forms to potential customers in the most convenient/efficient way, enabling us to monitor the progress and make it as easy as poss at both ends.

I've been sent documents via Docusign before but only to sign paperwork:

1) Would Docusign enable people to fill in own parts of a PDF form we send them (and then sign?)
2) Are there any alternatives to Docusign that are better?
3) Any idea what cost is? I have looked at their website but they want me to contact sales, don't really want to waste their time and mine if cost is unreasonable - we would only be sending a handful a week.

Have you looked at AdobeSign?

It's what I use for clients to sign accounts and tax returns.

They have clear pricing and I believe will do what you need https://www.adobe.com/uk/acrobat/pricing/business.html
