example of filled 64-8 form

Can someone please advice me where I can find a filled example of the 64-8 forms, I have got the original blank 64-8 form from HMRC It looks pretty straight forward but would like to see a filled version to make sure my client ones are filled correctly.

It's probably pretty difficult to find an example one online, purely due to client confidentiality. But they are very straightforward: fill out the agent details on the left hand side, then have a director/partner/trustee/company secretary complete and sign the top left hand section, along with ticking the boxes for each tax they want an agent to deal with for them.

Thank you both for your reply, agree I've looked at the form again looks straight forward just wanted to be sure. Apparently it's best not to file that form online as it takes longer to get anything back from HMRC, I shall do paper form instead.

The online authorisation is much quicker than the paper 64-8 route.
