Cost efficient affiliate management tool/software???

Hi All,

We are looking for tools/module to integrate to our site and run our own affiliate programme. We are not looking for affiliate network but tool/module to create our own one.

Basically, we need the module to
1. create form for people to apply as our affiliates
2. generate specific affiliate URL links
3. We can set up the commission rates. We would like to set different commission rates for individuals and associations. It will be % of sales for a certain period of time
4. Keep track of the commission so we can pay commission easily.

We are not keen to pay thousands for this. Any good and cheap option would be great. If there is any good open source module, it would be even better.

Any suggestions will be appreciated.
