Ok, I will put the first one in....

Finance.

You can put a form on your finance related website, and earn money from every enquiry that is made through the form.

This is how it works. The lead is then sold on to finance / mortgage / insurance brokers around the UK, and you get about 50% of what the broker pays for the lead.

Good, but hard to rank for.

Affiliate scheme can be done with many, many finance companies, they all pay about the same, apart from a few greedy beggers.

paaleads.com are one that will buy your leads off you (but you must use their own form).

Hotels - bookings.com. Although its a bit too compet. for the Affiliati these day. A good converter. If someone is searching for "hotel in Stoke" its probably because they want one.

One of the best paying networks has got to be www.moreniche.com - they offer a smaller, more focused selection of merchants (weight loss, adult and gambling) however the commissions are much better than other networks and the landing pages/creatives etc are excellent.

More Niche and EPN and both excellent (already recommended)..
In the education niche there are some great paying aff's like elearners which pays like 50-100 per free sign up depending on the volume.
In dating niche Double your dating ebook (they upsell a hell lot of things later on) is great..

Best one we did was on cash plans & life insurance. £20 for a submitted form asking for a quote on one and £35 for asking for both.

No idea on their conversion rates (as it was a per lead rather than per sale) or how much they resold the data but a broker I was dealing with at the time said they didnt even make that commission per sale let alone thinking of paying out that level per lead.
