A standing order "form" is simply a piece of paper with the necessary details in one place and not any "official" form and therefore you can make it look like what ever you want it to.

Most forms I have seen are more or less copies of the direct debit form but without the dd legal parts and your company logo instread

Now there's a nice little feature these forums could have! Set templates that can be downloaded (like the standing order form) for use by small businesses!

Although the things you probably need on the form (don't quote me though, I'm not a bank!) are:

Title
Name

Address
Postcode
Telephone
Email

Account Number
Sort Code
Name on Account

Starting on (probably best to allow at least a month)
Amount
Pay every (month, day, etc.)

Signatures
Date

---
To Manager (manager, bank name, address, postcode)

And maybe a reference number box for your "office use only" so you can photocopy it and tie it up to your accounts.

You dont actually need your own account name, it can be good practice to have your banks name and address on it as a check that you have the sort code correct.

It is also worth noting if you require them to have a reference on it so that you can tell from looking at statements who the payments have come from/ what is in connection with.

When we did ours for a tenent in one of our properties we put it together ourselves, just like what was mentioned above, had the tenent complete it then i photocopied it and dropped it off any their local bank, they were quite happy to take it, and i've had no problems with collection of payments
