At the moment, the only way I can see you might be able to do it, is to use the full version of Acrobat itself. I can't see any way of making an object (of any sort) clickable inside Designer.
If you created the PDF in Designer first, then edited using Acrobat, would that work for you?

I've had very good results with ASSP (http://assp.sourceforge.net/). It's probably best run on a separate box, but can work in conjunction with Exchange (etc).
