Web Based / Database Driven Receipt Application Required

Hello All,

My client requires a Web Based Receipt Application, which is mySQL Database Driven.

A brief description of my clients requirements are as below.

Issuing Receipts and having a day book list of all receipts.

The information to be entered manually, or selected from previous.
Name
Address
Payment for (ie rent, legal fees)
Amount

>From a drop down selection:-

How the payment is made (details should be changeable from backend), Options to be:

Cash
Personal cheque
Bank / Building Society Cheque

Automatically created entries:-
Receipt Number
Date and Time Received

Once the information is completed the receipt should be printed out twice, one above the other on a single piece of A4 paper. We will use our headed paper so that our client receives the top half with logo etc, (details should be changeable from backend)

The information should then be held in a table format showing all receipts for that period. As banking is not always available everyday, the form should work on a per period basis as apposed to a daily basis, with the form resetting after each period has been actioned and banked. The table should show the completed amount of money received and sub-totals relating to how much has been received in cash and cheque. The cheque totals should not be split between personal and bank or building society. One total of cheques is sufficient.

Finally as an off the wall idea, would there be any mileage in have a facility on the receipt, which would not be shown on the printout, to detail exactly how the cash was received as firstly this would serve as a check that the correct amount has been taken to agree with the total figure entered and also this would produce a listing on the table of what notes and coins had been taken in total which would assist with cashing up at the end of period.

Please can you provide costings of creating the above system. This will be hosted on a Linux Web Server, PHP5, and mySQL.
