Subcontractor Agreement/Contract

Hi there, we are a consultancy practice and we are likely to be needing to use subcontractors for an upcoming project.
The people we use will likely have their own limited companies set up, but may not.
We have a few clauses that we would like to include such as

• The subcontractor is not allowed to then subcontract their work
• They must be eligible to work in the UK
• They are not allowed to approach our customers independently

There may be one or two more but not much.
We are trying to investigate how/where to get a contract created that we can use for these people.
Cost is an important factor for us as we are a start up.
