To my knowledge there is no official pro-forma functionality in Quickbooks.
So, I created a regular invoice template and called it pro-forma with the title changed to pro-forma etc. Then when I want to create a pro-forma invoice I just create an invoice in the usual manner but using the pro-forma template. Then if you right click on the invoice header you can select "mark invoice as pending" and when you save it will exist in the account but will not be recorded in your books.
Finally, when you want to convert it to a regular invoice you just return to it, switch the template to your regular invoice. Right click on the header and there will be an option like "mark final" which will post it to your books etc.
Do a google search for "pro-forma Quickbooks" and look specifically for the links to the Quickbook forum. It's very useful.
