NCR pads required

I am looking for someone to design and supply some NCR pads. Preferably 50 to a pad. Black and white on the front and prefer sequential.
No fancy staples or binding the heat pressed glue is fine.
Will be regular ongoing purchases.
After quotes please both for a5 and a4
