Help form filling for Maternity Allowance

Sorry if this post waffles a bit...not sure if anyone can help...Please

I am completing (albeit extremely late) a maternity allowance form and need some help with it as my accountant is on holiday at the moment.

I am both the employee and the employer as I have a Ltd company and pay a minimum salary to allow personal allowance (the rest of earnings is made up of divis as I am the sole director). I do not have a copy of any payslips.

I need to:

1. create payslips or some other evidence to show pay and any deductions - as far as I am aware the only deductions are NI contributions.

2. I don't know the amounts I would have been paid so assuming I was paid to personal allowance can anyone tell me what that would have been for pre 6 Apr 08 and post?

3. Any idea how I go about creating SMP1 form?

Hopefully that will make sense to someone that can help (please)?
 
Please can anyone help with payslip template or personal allowance for 2007/2008 or 2008/2009?

You would need to talk to your accountant and get the required information. If you create you own payslip, that would be a fabrication of the documents.

Would you be able to print the payslip for OP without knowing pay?

Hi Matt,

Sorry you wasnt very clear, but thanks for clarification. Clearly one would need information to produce this, and this is calculated easily enough if the client has kept reasonable records. The unfortunate thing is it can be time consuming.
