Filling out an N349 form

I wonder if anyone could help.

I am having trouble with the red and green parts of this form:

1. What goes in the red section?
2. Does the green section include my judgement amount + the amount it will cost to file this form?

The red section will be the Court that issued the Judgment Order. The green section is the total sum of the Judgment Order that remains outstanding. It does not include the costs of filing the application. These will be added at a Final Hearing.

At the top of the form (top right):

Is the claim no. the same one I put in the first parapgraph (the original claim number) or is a new claim number assigned?

The Claim number will be the same as the one you have already been assigned i.e the claim number as stated on the Judgment Order.
