Feeling very dim...

Im wanting to set up adsense on a blog site, but am struggling to understand the guide. It is saying to alter the nameservers for the domain, but wont that affect the hosting and the site?! Surely the domain nameservers need to be pointed to the host for the site to be visable?

You're looking at adsense for domains which isn't what you want. You want adsense for content.

Go to set up and select adssense for content which is top of the list.
