First thing is to stop 'sugging' and get the request for contact details off of the survey. Next would be to look into market research techniques and discover the basic principles behind effective research. As mentioned, your survey will give you no useful data at all. I wouldn't know where to start telling you what is wrong with it. Maybe it would be best to employ a qualified market research consultant for a couple of hours to guide you, or chat to a marketing lecturer at your university.
