Most definately possible. I use my accountant for all of that but I am sure the Internet / companies house website would have all the details, and cost you a lot less too!
You need to complete a J30 stock transfer form which can be downloaded here then appoint your partner as a director (if not already a director) and then yourself resign as director. If transferring the shares for free there is no stamp duty to pay and form does not need to be sent to the stamp office.
