We use http://www.creativebanners.co.uk/
Not cheap by any standards (full set at all standard sizes were about £300)
But the quality has always been excellent and they provide you with the photoshop files so you can make changes later on if you want.
Although depending on your use of the banners i've found that banner advertising is becoming less and less important to our affiliate program than via content driven sales (to the point where im thinking of jacking in getting banners done for seasonal events like xmas, easter, fathers day etc)
