Affinity Cards

Does anyone know how to go about setting up an affinity card?

I know that banks and/or credit card companies work with you such as MBNA but I can not find details of anyone to contact about this. If anyone has any contact information of people who work with you to set them up, that would be great. I have just spent twenty minutes on the phone to MBNA trying to get their automated answer phone to put me through to a real person.
