We used to get this regularly until we bought a kimble gun and started tagging all our own printed garments. We only accept returns for wrong size/changed mind if the tag is still attached and it costs us under 1p per item for the tag and kimble barb. We haven't had this problem since - It also looks more professional in my opinion.
