Contact form please help

Hi guys,

We're trying to put a contact form in place in out of our site but been struggling to get it working, we had this problem before and Gary helped us to sort it out and we tried to cut and paste the same form but it's just not having it.

Everytime we clicked submit it comes back with loads of scripts ...???
