I was interested in their payment system rather than form. The form is quite long but makes sense for the information they need. Fortunately I found out the payment system was stripe :)

The name fits well, but you should still brainstorm for something catchier. Although this came top in your survey, the other names might've been awful, so I'd say that you should keep brainstorming to see if anything catchier pops up, and if not then go with Don't Plan Ahead!
