We started this a few years ago. I think we paid just over 2k for the initial set up then 1k for the annual audits. We had to do it as a charity, it was a real pain in a lot of areas but it did force us to make sure policies were reviewed/updated regularly.

She was dismissed for gross misconduct after less than 2 years. I don't know why she wants to see the reference, best guess is that she wants it for future jobs. We've never been asked to disclose a reference before.
She has asked to see the references from her previous employment. I was advised that we should tell her to ask for them from the previous employer, but don't we have to disclose these under GDPR?
No doubt there's something in the contract allowing them to do this, but I'm interested if extra charges for going concern work are a common occurrence. Many of our suppliers and partners have been very helpful (we're in the hospitality sector).
