If a websites are related enough to provide high quality traffic, then surely they will be competitors?
I think there will be very few circumstances where two businesses can swap ads and both receive high quality traffic which converts well.

Swap ads? This is just the same as link swapping, and will be rejected more bcos noone will give up their webspace freely - your only hope is to approach the desperate, or the sites that struggle or have no traffic to speak of.
But since these sites will have no traffic, there's no point in swapping links is there? -- unless you want to swap pagerank.... This seems like a new way to get links, but like the unknown, its tough to make people change their minds about what they already know or prefer.
Suggest you concentrate on simple link swaps, you'll pick up a few, but never the thousands many like to make out they got themselves.
The very few will work out how to do this correctly, BUT not every website is a link magnet, and its not as simple as what you think is of value.... its what others think!! Basically, a normal biz site will struggle, while a unique concept website will get thousands of links, so its about having that great idea, plus what you offer beyond a normal link for link deal.

Link exchanges, add exchanges or whatever you want to call it is a contentious issue and is one that is changing all the time.
I have developed and promoted many websites over the years and have come the conclusion that every site should be treated differently depending on its purpose and niche.
What you have to work out is "Can I provide my visitors with info on other relevant sites without harming my site or losing a potential customer.
Like I said this would largely depend on the sector.
Lets say your running a site for a legal firm. The benefits from any link exchange of any kind would be minimal.
However if like I am right now, you are running a car sales site your site and its visitors could benefit hugely by link exchanging with other related sites.
Having said that I don't mean linking to other car dealers. Links to Car accessory shops, Insurance sites, Driving instructors and so on would be beneficial.
Done right Link or add exchanges can work well but "think before you link"

How would an advert on an electrical wholesalers provide good quality leads for an electrician? Why would people who need an electrician be on a wholesalers website?
I said that not being in competition AND providing good quality leads is rare.

I would have thought doing ad swaps isn`t about increasing leads, it`s just a way to increase traffic to a site.
You don`t need to swap with competitors, we have a website selling residential property, so we could exchange with loads of related businesses, i.e. garden centres, builders merchants, kitchen designers, conveyancing, HIPs, etc.

Correct, linking is about driving traffic. A weblink cannot result in a sale, nor force a sale, although webcopy helps, its more about the web idea that makes websites valuable.
You have to get your biz site recognised first, then drive enough visitors to it, then you may make some sales - then again you may not. It's tough to stand out, as 95% of major websites are important well-known search engines that control how websites stand out (eg: Rank/Pos drop), their very costly in ad fees and with so many website competitors in each industry/web pages/junk sites/personal pages - its a joke how hard it is to stand out effectively enough, or for even hundreds to just find you.
Lets face it, a searchers life is not an easy one these days. The junk out there must be phenomenal
Even when I go after links (which isn't as often as I once did), the sheer junk I have to wade through to scout related websites is incredible, it shocks the hell out of me that the search engines allow this to happen in the first place.The web linking used to be great, I got tons of links years ago thankfully, now it's just crap, fighting to get maybe 3 decent links takes weeks sometimes
Now I just wait for sites to contact me, mostly it's unrelated sites begging for links, which is kind of worrying actually Requesting/Begging for links isn't the commodity it once was, that's the only guarantee you can be sure of. Money talks, bs walks....... The sooner people realise that if they want on other people's property, they must pay, the better for all concerned, as it will cut out all this desperate begging and seeing as link pages are just worthless and have been for a few years, there's not many options left.
Money talks and it gets you things, while who's going to rush to answer a link begging email.... I bet the offer of money gets you a result faster.

What's the point? You end up with traffic which doesn't convert to a single sale. You may as well have no traffic at all.
Think about it. Traffic on its own is useless. You need targeted traffic. This is made up of people who are interested in buying what you're selling. These are almost the only people who will buy from you there and then.
How many of the visitors who are visiting garden centres and kitchen designers will be really looking to buy a home? How many of those visitors are going to realistically look at your site and end up buying a house? Not many I imagine.
It's fine on its own as the ad won't be doing any harm. However, if you start advertising their site in exchange, then you're directing your targeted traffic away from your own site. Ironically enough, this traffic will probably be made up of many people who aren't that interested in what the other site has to offer either.
