I'm forming a company and the formation agent requires a 'certified copy of my passport or driving license' for the registered office. Do most lawyers do this? If so, what is the usual fee?

Also, apart from the photocopy on the A4 paper, what is the text that goes with it before the lawyer signs it? Is there a template?

Yes, most solicitors will do this for you. The form of word used is "We certify this to be a true and accurance copy of the original" but they will put this on the copy and then sign and date to say that they have compared it against the original.
There is no set fee for this, but it will be around the same rate as a swear at £5 per document.
