Does anyone have a standard type form they could let me have please that you give to clients asking for a period of notice if they no longer require your services?

Are you asking for a form to stipulate the requirement of a notice period - ie what you would expect to see in the T&Cs of the contract they sign - or do you want a form for a client to complete to actually give you the notice when they want to end their relationship with you?

This should be in your standard terms and conditions, your quote and/or your order acknowledgement.

Just include the words/terms you want

In normal circumstances I would agree but by nature of my relationship with the company in question its different..hence me asking for a form!

I am looking for a form for this company to sign that says I require two weeks notice if they no longer require my services.

Just write them a letter (two copies) one copy for them to sign and return.

Use businesslike wording that states that you (or both parties) agree that termination of your services requires ?? working days notice of which ever party is seeking to terminate the relationship.

I would not go for a formal looking form - a simple polite letter should be just fine.
