Hi guys,
I am looking for a free data capture form. ( or code for )
I am helpping a friend set up some very basic online advertising, at a very low budget, and basically want the advertising to take you to a very basic form that has a bit of copy at the top and then a data capture form.
the data i need is
name
address
telephone
email
message box.
it does not have to be connected to an autoresponder as he wants to follow up in person/over the phone, and then post out an opportunity dvd to them. ( If you are interested in a Uk based Home business opportunity-not kleeneze or avon! this is a shameless plug for you....Pm me)
Any help and or advice greatfully received.
