Microsoft Word - where do I go to create questionnaire format, facility for people to tick boxes etc?

Why use Word?
In any case, it just a matter of setting up the questions and adding some tick boxes so not quite sure what your problem is.
If it's adding the boxes then insert an empty text box or even draw some boxes or use a wingding character.
Another woud be to use a table for the questionaire and use cell borders to create the tick boxes.
Or you can use form fields.
Loads of way to do this.

If you go to microsoft office online there are various templates you can use to create questionnaires.

If you are lookign at under 20 quesitons and under 50 responses per day then I would use this one:

If you exceed those limits it will still continue to work but to get the results analysis you would need to subscribe (£20)

There are a bunch but we have used surveygizmo a good few times and it always works out. Easy to build the forms, handles all kind logic and branching options if needs be.
has a free option as well for low numbers and then monthly (cancel anytime) subscriptions.

Think this is the new version, but if you use the help index in your current word it should help, or just feel your way through the options.
I've create forms before and you just have to remember to protect the form so people can just tab along to the next box.
