What is needed to take credit/debit card payments over the phone.

A credit card merchant account from your bank (or any other bank for that matter although your own will know you and your business better) and a facility from someone like Protx, Velocity Pay, Secure Trading etc...

These providers will also give you the ability to take card payments through your web site...

In addition to the above you can use the secure payment page as Openmind suggested and input the card details, or you can use a device connected to your phone line, if you do not have a website. Your bank is the best place to start.
