What forms of lead generation do you use?

As some of you will know we are nearly ready to launch lead-generation-services to the market.

Before we do this, we would just like to check we have the major bases covered so would like to know

a: what forms of lead generation you use ( in order or priority if possible with 1 being the highest)
b: do you know what your average cost per lead is by each method?
c: Do you know you conversion rate i.e. how many leads you need per sale?

Yellow Pages currently standing at £400 (1 client so far)
