There is a very official looking form that I believe you can get from most stationers, it's folded A3 (so 4 sides of A4), and it details what a company owes you.

From what I understand you can send this to a company prior to commencing legal action to retrieve monies owed.

Does anyone know what it's called and where to get it?

There are legal bods on the forum but the basic rule of thumb is :

1. Invoice
2. Statement
3. Chase call
4. Chase letter
5. Final demand
6. Legal

A lot of people complete the form then send a copy to the debtor saying they will be filing it at court if payment is not made within x days
