I have a working understanding of how & why webpages appear ranked as they do on google. However, how does google rank images when you search by image?

Or, put another way, what do I need to do to get my images appear on page 1 of a google image search?

Name your images with a descriptive name.
Sign in with Google webmaster and select the option for "Include my site in Google Image Labeler" when your site is set up with it.

On a similar theme - how do you stop people nicking your images from Google images?

^ You can't.

Use Alt text as well as Tooltip text - there's two opportunities there to maximise your chances of being found. :)

You can't stop people nicking them, but you can stop them appearing in a Google image search.

Place this in your robots.txt file

User-agent: Googlebot-Image
Disallow: /
